Eligibility: Period Awards eligible for recommendation must have been issued between May 1, 2025 (Thursday) and April 30, 2026 (Thursday). Eligibility is based on the date printed on the award certificate or diploma.
Application Requirements: Candidates meeting the criteria in the “Award Threshold Standards Table” are encouraged to apply. Please provide documentation as follows ● Categories 1–27 and 29–38: Must submit proof of achievement (e.g., award certificates/documents) for review. ● Categories 28 and 39–70: Must submit proof of achievement, competition regulations (rules/brochures), and an official program (schedule) demonstrating that the competition included participants from at least six counties/cities.
Submission Process: Please complete the recommendation application form and place it, along with the required documents, into an envelope (attach the provided “Cover Page” to the outside) Deadline: April 30, 2026 (Thursday) by 5:00 PM. Methods: Deliver in person or via registered mail to Fongan Elementary School, Mailiao Township (No. 227, Houan Village, Mailiao Township, Yunlin County, 638). Note: Submissions are based on the postmark date. Late or incomplete applications will not be accepted.
