I. Application Documents (see Item 9 of the Selection Guidelines for details)

1. “Year 115 Outstanding Youth Candidate Recommendation Form” and “Self-Assessment Form,” with supporting documents (see Annex 2).
2. Autobiography/CV (see required contents in the Selection Guidelines).
3. Letter of Recommendation.
4. Official academic transcript(s) for all semesters during your study.

二、申請方式：繳交紙本及電子檔資料各一份

II. Submission Method: submit one hard copy and one electronic copy

1. Hard copy: submit to the Extracurricular Activities Section, attn. Mr. Ke-Ju Lin.
2. Electronic copy: email to kegi0712@ntnu.edu.tw.
   (Email subject: 【Application—Year 115 Outstanding University Student】Full Name)

III. Application Period: From now until 17:00, Friday, January 2, Year 115 (Minguo calendar; i.e., Jan 2, 2026). Late submissions will not be accepted.
